This form is the best way to request an appointment, and I truly appreciate you using it- It helps me stay organized and respond as efficiently as possible.  I typically reply to requests weekly, so hang tight, and I'll get back to you as soon as I can.   Here are a few important details to know before submitting your request.

My hourly rate ranges from $150-$200, with a 1 hour minimum.

A non-refundable $100 deposit is required to secure your appointment.  This amount will be applied towards the final cost of your tattoo.  Deposits may be transferred to a rescheduled appointment if at least 24 hours notice is given.

To help bring your vision to life; please provide as much detail as possible while describing your design idea.
